    To provide footwear evaluations, fittings, orthotics, follow up care, and to communicate with your healthcare provider (for example, obtaining prescriptions or documentation required by Medicare or insurance).

  • For Payment

    To verify eligibility, obtain prior approvals, and submit claims to Medicare or your insurance company.

  • For Healthcare Operations

    To improve quality of care, train staff, maintain records, or audit internal processes.

  • Required by Law

    We may disclose information if required by federal or state law, including public health reporting, audits, or legal requests.

  • To Business Associates

    We may share PHI with vendors who help us operate (orthotic labs, secure software providers). All business associates must comply with HIPAA.

• Most uses of psychotherapy notes (not applicable to our services) unless you give us written authorization.

Receive a list of certain disclosures we made outside of treatment, payment, and operations.
